The system can be set to execute a command on receipt of an SMS, for example mailto:fred@home.com and this will relay the SMS to an SMTP recipient. That part works fine but we would like to send a reply to the sender of the original text to say the SMS had been received as well as forwarding the message to SMTP. Can the 'command to Execute' box in the 2-way tab be set to execute both of these actions?

No, there is not a way to do this. A single message can only match one "command to execute".

An auto-responder on the mailbox might be the easiest way to do this (assuming you have things enabled to allow the e-mail receiver to reply back to SMS).
